WHAT TO EXPECT

Results, stage by stage.

LDN works cumulatively as the endorphin system recalibrates. Expect a gradual build rather than an immediate shift.

WEEKS 1–2

Starting dose: 1.5mg (1 capsule)

Take at bedtime. Some patients notice mild sleep changes in the first week — vivid dreams or slightly disrupted sleep — as the body adjusts to the brief overnight receptor occupancy. This is a common and temporary adjustment that resolves for most patients. Side effects are typically mild and transient.

WEEKS 3–4

Increase to 3mg (2 capsules)

Most patients tolerate this transition well. A subtle shift in energy, mood stability, or reduced inflammatory flaring often begins to appear in this window. Sleep typically stabilizes. Endorphin production is building.

WEEKS 5–6 ONWARD

Maintenance at 4.5mg (3 capsules)

The therapeutic target for most patients. For those with confirmed autoimmune diagnoses, 4.5mg is the clinical goal as the optimal endorphin rebound dose. Meaningful changes in immune regulation, inflammatory markers, pain levels, and sleep quality are typically reported here. LDN has demonstrated significant improvements in pain, quality of life, and inflammatory markers across multiple chronic conditions in clinical studies.

MONTHS 3–6

Sustained immune recalibration

The cumulative anti-inflammatory effect builds over time. Autoimmune flare frequency and severity typically reduce. For patients using LDN for longevity and cellular health, the ongoing endorphin stability and reduced neuroinflammation represent a long-term investment rather than a short-term fix.

Every patient's context is different. Some notice significant shifts within 4–8 weeks; others experience gradual improvement over 3–6 months. LDN is generally well-tolerated — fewer than 10% of patients report side effects, and most are mild and resolve within the first few weeks. Consistent nightly use is the single most important factor. If you miss a dose, simply resume the following night — do not double up.

ABOUT THE TREATMENT

What is Cell Renew Capsules (1.5mg)?

Understanding why inflammation persists

In autoimmune conditions, the immune system produces antibodies against the body’s own tissues — the thyroid in Hashimoto’s, the joints in rheumatoid arthritis, the nervous system in MS. In chronic inflammatory conditions, the underlying driver is often neuroinflammation — a persistent activation of the brain’s immune cells that conventional anti-inflammatories reach poorly. In both cases, the immune system hasn’t simply overreacted. It has lost its regulatory calibration.

Low dose naltrexone works upstream of this process.

How the endorphin rebound works

At standard doses (50mg), naltrexone occupies opioid receptors continuously — blocking them completely. At low doses (1.5mg–4.5mg), something entirely different happens. LDN briefly occupies the body’s opioid receptors for a few hours overnight, then clears. The brain interprets this as a signal to produce more of its own endorphins — a rebound effect that elevates endorphin levels above baseline and sustains them throughout the following day.

Endorphins are not simply pain-relieving compounds. They are central regulators of the immune system. When endorphin levels stabilize, the misdirected immune activity that drives autoimmune conditions — the antibodies attacking the body’s own tissue — begins to settle. The immune system recalibrates toward its appropriate function rather than being suppressed. LDN modulates immune activity toward normal function. It does not suppress immunity.

The neuroinflammation mechanism

LDN acts directly on microglia — the immune cells of the central nervous system — reducing their activation and the neuroinflammation that underlies brain fog, cognitive heaviness, and the mental fatigue associated with autoimmune disease, long COVID, fibromyalgia, and chronic Lyme. LDN has demonstrated significant reductions in glial cell activation and pro-inflammatory cytokine levels in the CNS in preclinical and clinical research.

Cellular aging and cancer risk reduction

When cells are unable to repair themselves — because the inflammatory environment or cellular signaling has been disrupted — they age faster, accumulate mutations, and occasionally replicate incorrectly. LDN’s endorphin-stabilizing effect supports the cellular repair signaling that keeps healthy cells renewing rather than degrading. For patients with a family history of cancer or elevated inflammatory markers, this makes LDN a meaningful preventive protocol alongside any other cancer risk reduction strategy.

Sleep

When stress hormones fluctuate dramatically — often the case in chronic inflammatory conditions — stable sleep becomes difficult. As endorphins stabilize and neuroinflammation decreases, many patients find that sleep improves substantially. This is the downstream result of a calmer neuroendocrine environment.

HOW TO USE

Four steps, every day

  1. STEP 01

    Start at 1.5mg — Weeks 1–2

Take 1 capsule by mouth at bedtime. Bedtime timing is clinically significant — LDN’s brief receptor occupancy aligns with the body’s natural overnight endorphin cycle, maximizing the rebound effect.

  2. STEP 02

    Increase to 3mg — Weeks 3–4

Take 2 capsules at bedtime. Most patients tolerate this step smoothly. If you notice significant sleep disruption, give your body a few extra days before continuing to increase.

  3. STEP 03

    Maintain at 4.5mg — Week 5 onward

Take 3 capsules at bedtime. This is the therapeutic maintenance dose for most patients. Patients with a confirmed autoimmune diagnosis should aim for 4.5mg as their clinical target — your HerTeleMeds clinician will guide you on the right timeline.

  4. STEP 04

    Consistency is the protocol

LDN works through an endorphin rebound mechanism that requires consistent nightly administration. Taking it at the same time each night supports the stability of the effect. Missing a dose occasionally is not a problem — simply resume your normal schedule the following night without doubling up.

Critical: Do not take if you are currently using opioid medications. LDN and opioids act on the same receptors. Taking them together will prevent LDN from working and can precipitate opioid withdrawal. This includes prescription pain medications, methadone, and buprenorphine.

LDN has demonstrated significant improvements in pain, quality of life, and inflammatory markers across fibromyalgia, Crohn’s disease, multiple sclerosis, and other chronic conditions in clinical studies.

LDN modulates immune function toward normal — it recalibrates antibody activity without suppressing the immune system, leaving the body able to fight infection normally.

LDN reduces microglial activation and neuroinflammatory cytokine levels in the CNS — addressing the brain fog, cognitive fatigue, and mental heaviness that accompanies chronic inflammation.

Titration from 1.5mg to 4.5mg over 6 weeks — a protocol designed to let the body adapt rather than shock the system.

FAQ

Questions, answered.

What is low dose naltrexone and how is it different from standard naltrexone?

+

Naltrexone at standard doses (50mg) is used to treat opioid and alcohol dependence by fully and continuously blocking opioid receptors. At a fraction of that dose — 1.5mg to 4.5mg — it produces completely different effects. LDN briefly occupies opioid receptors overnight, triggering an endorphin rebound that elevates the body’s own endorphin production. Those elevated endorphins then regulate the immune system and reduce neuroinflammation. This mechanism is well-researched, though LDN remains an off-label prescription.

I’ve been offered methotrexate, hydroxychloroquine, or a biologic like Humira. How does LDN compare?

+

Conventional disease-modifying drugs for autoimmune conditions work primarily through immune suppression — reducing overall immune activity to limit the autoimmune response. LDN works differently: it recalibrates the immune system toward normal function rather than suppressing it. LDN modulates glial cell activation and pro-inflammatory cytokine production without the systemic immune suppression associated with conventional immunosuppressants. This means the body retains its ability to fight infection, doesn’t carry the organ toxicity risks associated with long-term methotrexate or biologic use, and doesn’t require the monitoring protocols those medications demand. LDN is typically used as a complementary or alternative option — discuss the right approach for your condition with your HerTeleMeds clinician.

I’ve been on prednisone cycles for years. Can LDN help me reduce or stop them?

+

Many patients use LDN specifically to reduce their reliance on steroid cycles. LDN addresses the underlying inflammatory dysregulation that makes repeated prednisone courses necessary — rather than treating each flare as it occurs. Transitioning off steroids should always be done with clinician guidance; LDN does not replace urgent steroid use but may reduce how frequently that use becomes necessary. Discuss your current prednisone protocol with your HerTeleMeds clinician before starting.

I’ve been doing curcumin, fish oil, berberine, and other anti-inflammatory supplements. Why would I move to a prescription?

+

Functional medicine supplements offer real support for mild inflammatory conditions. For patients whose inflammation has not responded sufficiently to OTC protocols — or whose autoimmune condition requires more targeted intervention — LDN works at the receptor and neuroimmune level with a precision that supplements can’t replicate. LDN has demonstrated measurable clinical improvements in conditions including fibromyalgia, Crohn’s disease, and MS in published studies — outcomes that most OTC stacks have not been able to produce in clinical research. The two approaches can also coexist; LDN does not interfere with most supplements.

Who is a good candidate for Cell Renew Capsules?

+

Women managing autoimmune conditions (Hashimoto’s, lupus, fibromyalgia, MS, chronic Lyme, CIRS), those experiencing persistent neuroinflammation or unexplained brain fog, women with long-COVID symptoms, those pursuing a proactive longevity and cancer-risk-reduction protocol, and patients with sleep disruption linked to inflammatory or stress-hormone patterns. A consultation with your HerTeleMeds clinician will confirm whether LDN is appropriate for your specific context.

Why can’t I take LDN if I’m on opioid medications?

+

LDN occupies the same opioid receptors as prescription pain medications, methadone, and buprenorphine. Taking them simultaneously prevents LDN from functioning and — more significantly — can precipitate opioid withdrawal. This is the one absolute contraindication for LDN. If you are currently taking opioid medications, LDN is not appropriate at this time.

How long before I notice results?

+

Most patients begin to notice meaningful changes within 4–8 weeks, with fuller effects building over 3–6 months of consistent use. LDN is a cumulative protocol — it works best when taken consistently and patiently. Some patients notice sleep and mood improvements earlier; autoimmune symptom reduction tends to take longer.

Will this affect my thyroid medication?

+

Possibly — and this is a positive sign. For patients with Hashimoto’s, LDN may reduce thyroid antibody levels as immune function recalibrates. This can mean your thyroid medication dose needs adjustment. Your HerTeleMeds clinician will monitor thyroid function and adjust your protocol accordingly.

Chronic inflammation has a mechanism. So does the answer.

Cell Renew Capsules are for women who have done the research, asked the questions, and deserve a clinician who can explain what’s actually happening in their body — and prescribe accordingly. LDN works at the level of endorphins, immune receptors, and neuroinflammation. A HerTeleMeds clinician will review your history, confirm whether this protocol is right for you, and help you understand the mechanism behind the prescription.
