Full Hair Growth Serum (Gentle) — Finasteride 0.1% + Minoxidil 5% + Ketoconazole 2% + Azelaic Acid 12.5%

Four mechanisms. One formula. A gentler path back to your hair.

Full Hair Growth Serum (Gentle) combines four clinically studied hair loss actives in a single compounded topical — each targeting a different mechanism in the androgenetic hair loss cascade, from vasodilation and DHT inhibition to androgen receptor blocking and scalp environment support. Formulated at a gentler finasteride concentration for patients who want a comprehensive prescription approach without the intensity of higher-dose protocols, this is the all-in-one that doesn’t ask you to choose between effective and tolerable.

WHAT TO EXPECT

Results, stage by stage.

Hair regrowth from any therapeutic protocol is measured in months, not weeks. FPHL treatments work by arresting progression and gradually improving follicle health — expect to see initial changes in shedding before visible regrowth.

WEEKS 4–8

Reduced shedding

The first and often most important early signal. Minoxidil and ketoconazole begin improving scalp circulation and environment relatively quickly. Many patients notice that active shedding slows before new growth becomes visible — this is a meaningful early response.

MONTHS 3–4

Initial regrowth

By the end of 16 weeks, significant improvement on the Sinclair scale was observed in patients using topical finasteride and topical minoxidil respectively in a published clinical trial. Finer, shorter hairs at the hairline and part line are a positive sign that follicles are responding. Continued use is essential.

MONTHS 6–12

Meaningful density improvement

Consistent, sustained use produces cumulative results. Hair diameter and density improve as miniaturized follicles recover over multiple growth cycles. Most patients reach their most significant improvement at 6–12 months of continuous use.

BEYOND 12 MONTHS

Long-term maintenance

FPHL is a progressive condition. Discontinuing treatment typically results in renewed hair loss over time — the goal of treatment is both recovery and maintenance. Your HerTeleMeds clinician will advise on ongoing protocol.

Individual response varies significantly based on how long FPHL has been progressing, hormonal profile, and degree of follicle miniaturization. Earlier intervention generally produces better outcomes — follicles that have fully miniaturized over many years respond less completely than those at earlier stages of loss. Consistent daily use and SPF (which reduces UV-driven inflammation that worsens FPHL) are the two supporting factors most within a patient’s control.

ABOUT THE TREATMENT

What is Full Hair Growth Serum (Gentle)?

Why female pattern hair loss needs more than one active

Female pattern hair loss (FPHL) — also referred to as androgenetic alopecia in women — is driven by a cascade of overlapping mechanisms: androgen-driven follicle miniaturization, reduced scalp blood flow to affected follicles, scalp inflammation, and fungal/seborrheic dysregulation that worsens the follicular environment. Addressing only one of these with a single active leaves the others untreated. Full Hair Growth Serum (Gentle) was compounded to address all four simultaneously.

How each active works

Minoxidil 5% is the only FDA-approved topical treatment for female pattern hair loss. It works primarily as a vasodilator — widening blood vessels in the scalp to increase blood flow and nutrient delivery to hair follicles, extending the anagen (growth) phase and supporting follicle health. At 5%, it delivers a stronger growth signal than the standard 2% OTC formulation that most patients have already tried.

Finasteride 0.1% inhibits 5-alpha-reductase, the enzyme responsible for converting testosterone to dihydrotestosterone (DHT) — the androgen that drives follicle miniaturization in pattern hair loss. Clinical research shows topical finasteride is as safe and effective as topical minoxidil in FPHL. At 0.1% — a lower concentration than most compounded finasteride protocols — it targets DHT at the scalp with a gentler systemic exposure profile than higher-dose formulations.

Ketoconazole 2% is an antifungal with anti-androgenic properties that addresses two contributing mechanisms at once: it reduces scalp fungal burden (which drives the inflammation that accelerates follicle miniaturization) and has direct anti-androgenic activity at the androgen receptor. Topical 2% ketoconazole has achieved hair-stimulating effects comparable to 2% topical minoxidil in FPHL patients in published research.

Azelaic Acid 12.5% is a 5-alpha-reductase inhibitor that works through a complementary mechanism to finasteride — reducing DHT production at the scalp while also providing anti-inflammatory and antibacterial support that improves the overall follicular environment. With a high bioavailability and superior toxicological profile compared to many hair actives, it is well-suited for inclusion in a formula targeting patients with sensitive systems.

Why the “Gentle” formulation

The finasteride concentration in this formula is 0.1% — lower than the 0.25–1% concentrations used in other compounded topical finasteride protocols. For patients whose clinician has assessed them as better suited to a lower finasteride concentration — due to sensitivity, tolerance profile, or individual health history — this formulation provides the multi-mechanism benefit with a gentler finasteride dose.

HOW TO USE

Four steps, every day

  1. STEP 01

Apply to clean, dry scalp at night

Apply 0.5–1 dropperful directly to the scalp — not to the hair — focusing on areas of thinning. Parting the hair in sections to ensure scalp contact improves absorption. Apply at night to allow the formula to absorb without UV degradation of actives.

  2. STEP 02

    Massage gently into the scalp

Use fingertips to gently massage the serum into the scalp for 1–2 minutes. This improves absorption and supports the vasodilatory effect of minoxidil. Wash hands thoroughly immediately after application.

  3. STEP 03

Do not wash hair for at least 4 hours

Allow the serum to absorb fully before washing. Many patients apply at bedtime to allow overnight absorption, then wash hair in the morning if preferred.

  4. STEP 04

    Be consistent — every day

Hair growth actives work cumulatively and require consistent daily use. Skipping applications delays results. Set a reminder if needed — same time each night is the most effective protocol.

Finasteride transfer risk: Women should not handle this product if pregnant due to the possibility of skin absorption and the potential risk to a male fetus. Compounded topical finasteride products do not have the protective coating of FDA-approved finasteride tablets. Keep this product away from pregnant women and women who may become pregnant. Wash hands immediately and thoroughly after every application.

Four mechanisms in one formula: vasodilation (minoxidil), DHT inhibition via 5AR (finasteride + azelaic acid), androgen receptor blocking (ketoconazole), scalp environment support (ketoconazole + azelaic acid)

Minoxidil 5% — stronger growth signal than the standard 2% OTC most patients have already tried

Topical 2% ketoconazole achieved hair-stimulating effects comparable to 2% minoxidil in FPHL patients in published research

Finasteride at 0.1% — gentler concentration for patients whose system benefits from a more measured approach

FAQ

Questions, answered.

Why does female pattern hair loss need four different actives?

+

FPHL is driven by multiple overlapping mechanisms — DHT-driven follicle miniaturization, reduced scalp circulation, scalp inflammation, and fungal dysregulation that worsens the follicular environment. Most OTC treatments address only one. Minoxidil improves blood flow to follicles. Finasteride inhibits the enzyme that converts testosterone to DHT. Ketoconazole provides additional anti-androgenic activity and improves scalp health. Azelaic acid adds a second DHT-blocking mechanism through a complementary pathway and supports the anti-inflammatory environment follicles need to recover. Together, they address FPHL from four angles simultaneously.

I’ve already tried OTC minoxidil 2% and it didn’t fully work — how is this different?

+

Several ways. The minoxidil in this formula is at 5% — a stronger concentration than the standard OTC formulation most patients try first. More significantly, this formula adds DHT inhibition (finasteride + azelaic acid) and scalp environment optimization (ketoconazole) that OTC minoxidil alone doesn’t provide. Topical ketoconazole 2% has been shown to achieve hair-stimulating effects comparable to 2% minoxidil — pairing it with 5% minoxidil and two DHT-inhibiting actives addresses the mechanisms that explain why minoxidil alone produces partial or insufficient responses for many patients.

What makes this the “Gentle” version?

+

The finasteride concentration in this formula is 0.1% — lower than the 0.25–1% concentrations used in other compounded topical finasteride protocols. For patients whose clinician has assessed them as better suited to a lower finasteride concentration — due to sensitivity, tolerance profile, or individual health history — this formulation provides the multi-mechanism benefit with a gentler finasteride dose.

Is topical finasteride safe for women?

+

This is an important question that deserves a direct answer. Finasteride is contraindicated for women who are or may become pregnant — the drug can cause serious birth defects in a male fetus when absorbed through the skin. For women who are not pregnant and not planning to become pregnant, topical finasteride has been used off-label for FPHL with clinical evidence supporting its efficacy. The FDA issued a safety alert in April 2025 noting that compounded topical finasteride carries systemic absorption risk and has been associated with adverse events in some patients. Every HerTeleMeds prescription for this formula is preceded by a thorough clinical consultation to discuss these risks honestly.

How long before I see results?

+

Reduced shedding is typically the first noticeable change, often within 4–8 weeks. Initial regrowth — finer, shorter hairs at areas of thinning — typically appears by months 3–4. Meaningful density improvement is most significant at 6–12 months of consistent use. Hair growth requires patience; the underlying biology operates on the timescale of hair growth cycles, not weeks.

Can I use this alongside other HerTeleMeds products?

+

Discuss your full routine with your HerTeleMeds clinician. This serum is a comprehensive nighttime topical — most patients do not need to layer additional hair actives on top. If you are using a topical for your scalp and separate skincare for your face, ensure the hair serum stays on the scalp only and does not contact facial skin.

Will my hair loss come back if I stop?

+

Yes — FPHL is a progressive condition and treatment effects are not permanent. Discontinuing typically results in renewed hair loss over time, as the underlying androgenetic process resumes. Many patients use a maintenance protocol long-term. Your HerTeleMeds clinician will advise on the right ongoing approach for your situation.
