WHAT TO EXPECT

Results, stage by stage.

Luminant works in phases — and the phases overlap. Each active in this formulation follows its own timeline, and together they build on each other. Expect an adjustment period first.

WEEKS 1–4

Retinization

The first 2–6 weeks of tretinoin use generally involve the skin getting used to it. During this time, it is common to experience some skin irritation, purging, dryness, and peeling. This is the retinization phase — the skin is responding, not reacting negatively. The TXA and niacinamide in the formulation actively support barrier function during this window, which is part of why a combination cream can be more tolerable than tretinoin alone. Starting at two nights per week as directed significantly reduces the intensity of this phase.

WEEKS 4–8

First visible shifts

After 4–6 weeks, skin begins to feel noticeably smoother and more even. Surface texture improves. Fresh dark spots and post-acne marks begin to lighten as TXA interrupts melanin production and niacinamide inhibits its transfer to the skin surface. The barrier is strengthening.

WEEKS 8–12

Meaningful improvement

Visible improvement is usually apparent by week 8. Clinical scoring in TXA + niacinamide combination studies showed a statistically significant 13% reduction in color intensity and 6% reduction in dark spot size at 8 weeks. Tone reads as more consistent. Pores appear tighter. Skin retains moisture more effectively as panthenol and niacinamide continue rebuilding barrier integrity.

MONTHS 3–6

Collagen remodeling

Studies show it takes 10–16 weeks of treatment to begin reducing wrinkles and fine lines, with up to 6 months before significant changes become visible. This is when the cumulative collagen remodeling driven by tretinoin becomes most apparent. Skin firmness and overall tone continue improving with sustained, consistent use.

Patients with inflammatory conditions like eczema or rosacea, a compromised skin barrier, or those who are not diligent about sun protection may not be well-suited for tretinoin. Some patients with these conditions experience a paradoxical worsening rather than improvement. Luminant is a strong starting place for most patients, but your HerTeleMeds clinician will assess your skin history before prescribing. If you have had difficulty tolerating retinoids before, flag that in your consultation.

ABOUT THE TREATMENT

What is Luminant All-In-One Face Cream?

What four actives accomplish together

Most prescription skincare addresses one concern at a time. Luminant was formulated around a different premise: that the most common skin concerns — uneven tone, dark spots, fine lines, dehydration, and barrier dysfunction — share underlying mechanisms, and a formula that addresses all of them simultaneously produces better outcomes than four separate products fighting the same battles on different schedules.

Tretinoin 0.05%

Tretinoin 0.05% is the backbone of the formulation. A prescription-only retinoid in its active form, tretinoin works directly at the cellular level — accelerating cell turnover, stimulating collagen production, and clearing the congested, irregular surface that makes skin look dull and aged. In clinical research, 0.05% formulations used over several months demonstrated measurable changes in photoaged skin — improved firmness, tone, and texture. Tretinoin also actively fades post-inflammatory hyperpigmentation by accelerating the turnover of pigment-laden cells.

Tranexamic Acid 3%

Tranexamic Acid 3% works upstream of pigmentation itself — interrupting the communication between the keratinocytes that respond to inflammation and the melanocytes that produce excess melanin. Where tretinoin clears pigmented cells from the surface, TXA reduces the production of new pigment at its source. Together, they address dark spots, age spots, acne marks, and uneven tone through two separate and synergistic mechanisms.

Niacinamide 4%

Niacinamide 4% is the formulation’s barrier architect. A B-vitamin that works inside the skin cell, niacinamide helps cells use their energy more effectively, regulates sebum production to keep pores clear and tight, and inhibits the transfer of melanin to the skin’s outermost layer. A 2019 study confirmed that TXA combined with niacinamide was significantly more effective than control regimens at reducing facial pigmentation by week 4. A stronger barrier also means the inflammatory response at the skin’s surface is reduced — which is why patients with redness and uneven tone often see improvement even when inflammation wasn’t the presenting concern.

Panthenol 3%

Panthenol 3% does something deceptively important: it attracts moisture and locks it into the cell. Panthenol (provitamin B5) is a humectant and emollient that supports barrier repair and reduces the transepidermal water loss that tretinoin can cause during the retinization phase. Its presence in this formulation is what makes an all-in-one approach with tretinoin sustainable — supporting the skin’s hydration actively while the other actives do their work.

Who this is for

Luminant is well-suited for women managing multiple concerns at once — fine lines and dark spots, uneven tone and dehydration, or post-acne marks alongside early signs of aging. It’s an especially strong fit for patients who have been managing several separate prescriptions and want to consolidate, or for those starting a comprehensive prescription skincare routine for the first time.

Luminant is not recommended for patients with active eczema, uncontrolled rosacea, or a significantly compromised skin barrier. For these patients, Tender Clear Cream (TXA 6% + Niacinamide 2%) offers a gentler entry point without the retinoid. Your HerTeleMeds clinician will determine the right fit.

HOW TO USE

Four steps, every day

  1. STEP 01

Apply to clean, dry skin at night only

Tretinoin degrades with UV exposure — Luminant is a nighttime-only formula. Cleanse, pat completely dry, and wait a few minutes before applying. Damp skin can increase absorption and amplify irritation.

  2. STEP 02

Start at two nights per week

Begin with two nights per week for the first two weeks. Starting slowly is not optional — it’s the protocol that allows the retinization phase to be tolerable and gives your skin time to adjust before increasing frequency.

  3. STEP 03

Increase by one night every two weeks as tolerated

After two weeks at two nights, increase to three nights. After two more weeks, increase to four, and so on toward nightly use — or the frequency your HerTeleMeds clinician advises. If irritation is significant at any stage, hold at your current frequency rather than continuing to increase.

  4. STEP 04

    Follow with a gentle moisturizer

Panthenol in the formulation supports hydration, but adding a simple, fragrance-free moisturizer over Luminant helps further offset the dryness that can accompany retinization — especially in the first few weeks.

  5. STEP 05

SPF 30+ every morning without exception

Tretinoin increases sun sensitivity significantly. This is the single most important supporting step — UV exposure works directly against tretinoin’s collagen-building effects and TXA’s pigment correction.

Do not use during pregnancy. Tretinoin is contraindicated in pregnancy. If you become pregnant while using Luminant, discontinue immediately and contact your HerTeleMeds clinician.

In a clinical study of TXA + niacinamide combination serum, melasma and hyperpigmentation improved an average of 60%, with post-inflammatory hyperpigmentation decreasing an average of 81% over 12 weeks

FAQ

Questions, answered.

Why does one cream contain four different prescription actives?

+

Because the most common skin concerns — fine lines, dark spots, barrier weakness, and dehydration — don’t operate independently. Tretinoin drives cell renewal and collagen production. TXA interrupts melanin production upstream. Niacinamide inhibits melanin transfer, regulates sebum, and strengthens the barrier. Panthenol attracts and retains moisture while supporting barrier repair. Each active addresses a different mechanism — and together, they produce outcomes that a single active cannot replicate alone.

What is the retinization phase and how do I get through it?

+

Retinization is the adjustment period that happens when the skin first encounters tretinoin. It is common to experience irritation, purging, dryness, and peeling during the first 2–6 weeks. Starting at two nights per week and building slowly is what makes this phase manageable. The niacinamide and panthenol in Luminant actively support the barrier during this window, which makes the formulation more tolerable than tretinoin alone. Most patients move through retinization within 4–6 weeks.

Can I use this if I have sensitive skin?

+

If you consider yourself to have sensitive skin but do not have an underlying inflammatory condition, tretinoin can be used if introduced carefully, slowly, and at appropriate low concentrations with a supportive skincare routine. Luminant at 0.05% is a moderate starting concentration — not the lowest available, but supported by the barrier-protective actives in the formulation. Discuss your sensitivity history with your HerTeleMeds clinician at consultation.

What if I have rosacea or eczema?

+

Patients with active eczema or uncontrolled rosacea should avoid tretinoin. Luminant is not the right fit for these patients in active flare. Tender Clear Cream (TXA 6% + Niacinamide 2%) is typically the better starting point for patients with reactive inflammatory skin conditions — your HerTeleMeds clinician will guide you to the right formulation.

How does this compare to using tretinoin and a separate serum?

+

Using multiple products in sequence introduces timing complexity, layering incompatibilities, and the risk of over-applying multiple actives. Luminant is compounded to precise therapeutic ratios specifically designed to work together — the panthenol and niacinamide concentrations are calibrated to support the tretinoin, not just coexist with it. One formula, once at night, removes the variables.

How does this compare to OTC alternatives like retinol serums or vitamin C + niacinamide stacks?

+

Retinol is an OTC precursor that the skin must convert to retinoic acid before it becomes active. Tretinoin is already in its active form — which is why it works more directly and more quickly, and why it’s prescription-only. OTC stacks can offer meaningful results for mild concerns, but they cannot match the clinical precision of a compounded, clinician-prescribed formulation at therapeutic concentrations.

Do I need to take a break from this product?

+

Unlike hydroquinone, tretinoin and TXA do not require scheduled treatment breaks. Many patients use tretinoin as a long-term maintenance protocol. Your HerTeleMeds clinician will monitor your progress and adjust frequency or concentration as needed over time.

Can I wear makeup after applying Luminant?

+

Luminant is applied at night — makeup is generally not worn over it. In the morning, after SPF, your regular routine can follow as normal.
